COMHAR’s Supported Independent Living (SIL) Unit encompasses several supportive residential housing settings all with various funding sources including local and federal stakeholders. The SIL programs support individuals with histories of homelessness, mental illness and co-occurring diseases like substance use disorder and chronic health conditions. For eligible members who are committed to their own recovery, the SIL programs provides subsidies to assist members with achieving independence in their own community integrated apartments or in our congregate style settings.

Duties and Responsibilities

  • Oversee all program-related activities
  • Assists the SIL Director with planning, organizing, and directing the operations of the department.
  • Assists the SIL Director with housing program evaluation and oversight of the U.S. Department of Housing and Urban Development (HUD) and Department of Behavioral Health and Intellectual disability Services (DBHIDS) funded programs.
  • Assists Director with supervising and training staff; provide direction and guidance in technical processes and procedures; 
  • Facilitate admissions and discharges. Review and respond to all DBH TIP referrals within 48 hours. For congregate setting admissions, coordinate with internal departments for compliance with their regulatory requirements.
  • When applicable, assist members with locating appropriate living spaces and assuring that necessary utilities are in place prior to move-in (I.e.- electric, gas, phone, etc), all in accordance with their individual lease agreement. Maintain history of lease compliance and document any ongoing issues of non-compliance.
  • Calculating member rent based on presented income and related HUD regulations; collecting the rent on a monthly basis
  • Facilitating the completion of unit inspections at least once per year and ensuring that all documentation is completed in a timely fashion.
  • Complete regular reporting as required for specific programs, to include, but not limited to, PDO’s for local stakeholders, HUD’s Annual Progress Report, CoC Renewal Application, Quarterly Reports, etc.
  • Recommends changes to policies and procedures for compliance.
  • Participate and facilitate regular concurrent review meetings with assigned DBH and OHS Program Analysts.
  • Generate work orders for maintenance issues; follow-up with submitted work orders to ensure completion
  • Maintain a chart for each resident that details the basic demographic information, necessary to assure their safety and recovery utilizing the established data systems (I.e.- Evolv, Client Track, HMIS, etc) and others deemed necessary for program maintenance.
  • Monitors operations and evaluates programs, services, and activities to assure they are meeting community needs; coordinates community meetings to discuss housing programs issues.
  • Ensure that SIL Counselors and other team members perform all duties necessary to the cases assigned to them in a thorough, appropriate and consistent manner, including but not limited to progress notes, treatment plans, in-person supports, collateral and collaborative supports and communication.
  • Supervise teams ensuring enrolled members are being assisted and supported with using resources to maintain their housing; develop support systems; and seek education, employment and vocational opportunities to support their recovery journeys.
  • Evaluate the performance of all assigned staff
  • Contributes to the budget process, understands fiscal issues and maintains service productivity level
  • Conduct and participate in supervision, staff meetings, team meetings, as well as meetings with housing partners, and funding/regulatory entities.
  • Represents the unit and agency in community (or other agency) meetings, events, outside programming, and outreach efforts.
  • Act as liaison between the owner of the property and the tenant, as needed.
  • Communicate with the assigned case manager and/or counselor of the tenant around any issue that could potentially hinder the consumer’s ability to remain in an independent living situation.
  • Respond to and act to resolve owner and program participant complaints regarding services of the housing program
  • Bring to the attention of the Program Director any issue which could jeopardize the status of a resident in the program or jeopardize a lease agreement.
  • Participates in all mandatory training at hire and annually, e.g., confidentiality, fire safety, infection control, crisis prevention/intervention, suicide prevention, cultural awareness, disaster training, driver training, medication monitoring, safety and emergency procedures, and other SIL/HUD procedures and practices, and others as assigned..
  • May be required to respond afterhours, including holidays and weekends, in the event of a departmental or City-wide emergency.
  • Assume all other duties as assigned by supervisor, CCSS Division Director as needed.
  • Driving is essential.
  • Experienced working with persons with SMI (Severe Mental Illness); members experiencing homelessness, members living with chronic health conditions and knowledge about regional HIV-specific resources preferred
  • Knowledge of support housing service provision and/or case management.
  • Master’s Degree in social work, psychology, or human service preferred.
  • Supervisory experience.
  • Minimum of two (2) year related experience AND three (3) years of progressive responsible experience managing multiple caseloads in housing, or social services programs.
  • Must possess a valid Pennsylvania Driver’s License.
