Site Manager Salary:
$21.00/HR or $43,680.00 annual salary
Site Manager Shift:
Monday-Friday 10:00AM-6:00PM
COMHAR is a nonprofit community-based organization. Our mission is to provide health and human services that empower individuals, families, and communities to live healthier, self-determined lives. We are seeking a Site Manager to join our Developmental Intellectual Disabilities program in Philadelphia, PA. The Site Manger will assist with day-to-day management of participants. The ideal candidate enjoys working in an ever-changing environment and helping those with disabilities.
Responsibilities:
- Coordination and/or completion of initial, annual and revision consumer assessment for assigned caseload
- Identifying in the assessment the consumer’s requirement for direct supervision or times when the consumer may be unsupervised
- Disseminating the assessment for the development of the IHP, IHP annual update and all IHP revisions to all IHP plan team members.
- Providing training to Direct Support Professionals on the consumer Support Plan if a medication is prescribed to treat symptoms of a diagnosed psychiatric illness prior to the Direct Support Professional working with the consumer
- Participating in the development of the IHP, IHP Annual Updates, and all IHP revisions
- Attending the IHP Annual Updates, and all IHP revisions
- Fulfilling the role of Plan lead when a Program Director is not available
- Completing quarterly reviews every 3 months (or more often if necessary)
- Reporting IHP content discrepancies to the Program Director as applicable and Team members
- Implementing the IHP as written
- Supervising, monitoring and evaluating services and outcomes identified in the IHP
- Supervision of all DSP members/agency staff
- Reviewing, signing and dating the monthly documentation of an individual’s participation and progress toward outcomes
- Reviewing all incident reports and signing all incident reports once completed and ensuring that the Program Director reviews each incident reports
- Reporting a change related to a consumer’s needs to the Program Director as applicable and to plan team members
- Completing all DSP working schedules
- Recommending a revision (addition, deletion, modification) to the service or outcome in the IHP when needed
- Coordinating the services provided to a consumer
- Coordinating the training of Direct Support Professionals in the content of health and safety needs relevant to the consumers on your caseload
- Developing and implementing component services that will lead to advancement Active Treatment
- Assure all facility van, physical site, and trainings are in compliance with DOH 6600 regulations
- Adhere to HIRS (Health Information Record Systems) standards in completing Medical and Programmatic Books.

Requirements:
- High School plus 4 years of supervisory experience working with intellectual Developmental Disability or Autism Or
- AA + 60 college credits in Human Service discipline + 2 years of supervisory experience working directly with Intellectual Developmental Disability or Autism.
- Must be proficient in Microsoft Office Programs (Word, Excel, etc)
- Valid driver’s license & own vehicle require
